My comments are for the local area of where I live, within 40 miles / 70 kms, so only need Canada. I edited includes/language/english/coutries.php so only Canada remains but all the countries still are in the list for posting a comment.
Do I have to edit the ones for the other countries too or can I just remove the directories for those languages?

It's /comments/includes/template/countries.php that you need to edit.

Also, if you only have one country, it might be better to disable the country field:

You're right, probably the easiest is just to disable the country field.
