Thanks for providing your access details via PM.

I had a look and I got the following page working:
_VA/CA/JT/2016/WH/JT16WtrHtr_comment.php

1. The first issue was with the integration code on that page. It was using the integration code from an older version of Commentics.
2. Your database was missing the 'geo' table.
3. Your 'Commentics URL' setting in 'Settings -> System' was incorrect.
