My website is a computer repair website, and I was wanting to know if commentics could be setup up like this page Request this service:
I need a request this service form on everyone of my different services page.

I also want to have a page for testimonials. Like the form at the bottom of this page. Submit your Testimonial:

I don't know anything about php but I'm pretty good at tinkering if given good directions.

Right now I think I can use what I have now on this page under a comment & questions page rather than a true testimonial page.

Hi,

You could quite easily use Commentics for a testimonials page like on that website, except it won't ask for their phone number or their physical address like they do.

But it sounds like you need those two things, especially if you want to use it for requesting a service too.

At the moment there is no support for additional fields. You could add this yourself (or hire a developer to add these for you if you're not good at PHP) but it's not something that I can help with right now.
